CARD REPLACEMENT

In order to get a replacement card you must write a letter stating the card was lost, stolen, or damaged. The letter must contain the following student information:

• Name
• Social security number
• Which test you took; 608 (HVAC) or 609 (Automotive)
• Mailing address and current phone number

There is a $10.00 fee that must be included. We accept Visa or MasterCard with the expiration date. Make money orders payable to UTI.
